“Consumer Health Records” (CHR) are those electronic records that are created in a form for direct ownership  and access by patients that contain information derived from the EHR data recorded by practitioners  in their provider organization systems and posted to the CHR in a form useful and relevant to the individual patient. The logical form and specific wording contained in the information posted may be different from that contained in the provider’s EHR. At this time there  are not common conventions for the content in such CHR systems. Nevertheless, the following categories may be included:

• Brief Demographics
• A List of Involved Practitioners
• A Summary Health History
• A Problem/Health Condition List
• Recent Physical Exam Observations
• Recent Laboratory and Other Diagnostics Observations
• Current Medications
• Dates and Details of Recent Encounters
• Scheduled Healthcare Appointments
